Guest Waiver It is expressly agreed that all activities and use of all facilities shall be undertaken by Guest at guest’s sole risk. Klohverleaf Dance Academy (KDA) and/or LiveNow Fitness, its affiliates, and/or its franchisee(s) and their authorized designees shall not be liable for any claims, demands, injuries, damages, or actions whatsoever to guest or guest’s property arising out of or connected with the use of any of the services and facilities of or the grounds on which the KDA activity is held or club is located (“Claims”). By signing below, Guest does hereby expressly forever release and discharge KDA and/or LiveNow Fitness, its affiliates, and/or its franchisee which owns the club, and the franchisee’s partners, agents and employees, from all such Claims. For mobile messaging, message and data rates apply and consent is not required to become a member. I agree to the Terms and Conditions and privacy policies. Media Release I understand, accept and grant permission for images/video/audio of myself/my child/my children to be published or exhibited for editorial and/or publicity purposes. I waive any right to royalties or other compensation arising or related to the use of the images/video/audio. I release KDA, its successors and assigns, its officers, directors, shareholders, employees and agents, and their heirs, executors, and administrators from any and all claims, including, but not limited to, those based on libel, invasion of privacy or violation of any right of publicity. The absence of, or lack of explicit mention to a specific claim does not limit the extent of the application of this agreement. I attest that I am at least 18 years of age and I accept and agree to the above terms on my own/my child's/my children's behalf.
